Contact Tracing Plan

  • This plan was developed in collaboration with and based on guidance provided by the Ontario County Department of Health. 

    1. The Phelps-Clifton Springs Central School District (school nurses and/or COVID-19 Safety Coordinator) will communicate with the Ontario County Department of Health regarding confirmed and suspected cases of COVID-19, as well as the appropriate steps to be taken for symptomatic individuals, positively diagnosed individuals, and proximate contacts.
    2. A student or staff member who displays symptoms of COVID-19 should stay home from school/work, indicate on the health screening form that they have symptoms, and follow-up with the school nurse. Symptomatic individuals will be encouraged to visit their primary care physician or an urgent care facility, with the recommendation to be tested for COVID-19.
    3. A student or staff member who displays symptoms of COVID-19 at school/work will be placed in a designated isolation space where they will be evaluated and supervised by one of our school nurses. Nursing staff will wear all of the designated and appropriate Personal Protective Equipment (PPE) when evaluating a potentially symptomatic individual. 
    4. In the case of a student, once symptoms are confirmed by a school nurse, the symptomatic student will remain in the isolation room until they are picked up by a parent/guardian. The parent/guardian will be encouraged to bring the student to visit their pediatrician/primary care physician or an urgent care facility, with the recommendation to be tested for COVID-19.
    5. In the case of a staff member, once symptoms are confirmed by a school nurse, the symptomatic staff member will remain in the isolation room until they are picked up by a loved one or can leave on their own accord. The staff member will be encouraged to visit their primary care physician or an urgent care facility, with the recommendation to be tested for COVID-19.
    6. In accordance with CDC guidelines, individuals who exhibit COVID-19 symptoms will not be allowed to return to school/work until:
      1. The individual tests negative for COVID-19 or receives written clearance from a physician/healthcare provider if they have been diagnosed with another condition; AND
      2. The individual has had no fever, without the use of fever reducing medication, and has felt well for at least 24 hours.  
    7. Individuals who exhibit symptoms but have not been tested for COVID-19 will not be allowed to return to school/work until:
      1. 10 days after the first onset of symptoms, and
      2. The individual has had no fever, without the use of fever reducing medication, for at least 3 days, and
      3. The individual’s respiratory symptoms have improved.
    8. In accordance with CDC guidelines, individuals who have tested positive for COVID-19 or have been diagnosed with COVID-19 by a healthcare provider will not be allowed to return to school/work until:
      1. It has been a least 10 days (or the number of days recommended by the local department of health at the time) since the individual first had symptoms; and
      2. It has been at least 3 days since the individual has had a fever (without using fever reducing medicine): and
      3. It has been at least 3 days (or the number of days recommended by the local department of health at the time) since the individual’s symptoms improved, including cough and shortness of breath; and
      4. The individual has received written clearance from the local department of health.
    9. The Phelps-Clifton Springs Central School District will coordinate closely with the local departments of health as it facilitates the contact tracing process. Using our daily attendance and class by class attendance, our student management system, our camera system, seating charts (bus and classroom), and/or visitor logs (if visitors are permitted), the District will work with the local departments of health to provide the information needed to conduct a proper and thorough contact tracing.
    10. The Phelps-Clifton Springs Central School District will follow the recommendation of the local health department regarding isolation, quarantine, and/or closure of classrooms or school buildings following a confirmed case of COVID-19.
