Digital Equity Survey

The Phelps-Clifton Springs Central School, in partnership with the New York State Education Department, is collecting data regarding the access all students have to digital resources with the goal of helping educators best service students and families.

As a result, the New York State Education Department is asking parents or guardians to complete a Digital Equity Survey for each student in their family in grades K-12. All Midlakes families are asked to complete the brief survey, which will provide information on student access to devices and internet access in the home.

To complete the survey, families will need their student’s ID, which can found on your student’s progress report or by logging into SchoolTool. When accessing the survey on a mobile device, click on "View Full Site" for the best experience. Thank you for your participation.
